Thank you for considering us for your COSMETIC DENTAL and FACIAL needs.  We do require $200 payment prior to scheduling a formal consultation.  You are paying for a private formal consultation meeting with Dr. Shieh to learn your treatment options for your smile transformation and/or facial injectable goals.  Please plan for an up to 2-hour visit and remove ALL facial and tongue jewelry prior to your arrival. 
 
Once we receive your payment, our team will email you our New Patient registration form link to complete online.  Once we receive your online forms, then we will reach back out to you to schedule your consultation.

You may reschedule one (1) time without penalty
Change of mind/withdraw from consult prior to scheduling, eligible for a refund of $150
Change of mind/withdraw from consult after scheduling, NO refund
After the 2nd cancellation, NO refund
$200 will not be credited to your treatment
Must have your consultation scheduled within 45 days from payment. If you fail to complete your consult within 45 days, then you will forfeit your $200 paid
