Dear Patient:
 
Thank you for considering Dr. Shieh for your Smile Transformation or Facial Esthetics.  We do require $250 payment prior to scheduling a formal consultation.  You are paying for a private formal in-person consultation meeting with Dr. Shieh to learn about your treatment options for your smile transformation and/or facial injectable goals.  Please plan for an up to 2-hour visit and remove ALL facial and tongue jewelry prior to your arrival. 
 
Once we receive your payment, you will receive an email with our New Patient registration form link to complete online.  Once we receive your online registration forms, then we will reach out and provide consultation dates/times for you to choose from.

You may reschedule one (1) time without penalty
Change of mind/withdraw from consult prior to scheduling, eligible for a refund of $150
Change of mind/withdraw from consult after scheduling, NO refund
After the 2nd cancellation, NO refund
$250 will not be credited to your treatment
Must have your consultation scheduled within 30 days from payment. If you fail to complete your consult within 30 days, then you will forfeit your $250 paid
