REGISTRATION

Once we've received your consultation payment, you will then receive an emailed link to complete your new patient registration

 

NOTE: Do NOT provide your SSN or driver's license number.

SCHEDULE

 

A team member will reach out to you within 24 hours via text, email or phone once we've received your online patient registration forms to help you select a day and time for your formal in-person consultation

MEETING

 

Get ready for your in-person consult:

  • Remove and leave all of your facial  piercings and jewelry at home (earrings, necklaces, etc.)

  • Minimal make up

  • Plan for an up to 2-hour meeting

Practice Communicable Diseases Safety & Prevention Policy

 

  • Practice Communicative Diseases Liability Waiver must be signed in your online registration in order for you to proceed with your visit

  • Mask upon entering to our practice is optional

  • Questionnaire may be conducted at your appointment and/or on your appointment confirmation call

    • Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms may have COVID-19:

      • Fever or chills

      • Cough

      • Shortness of breath or difficulty breathing

      • Fatigue

      • Muscle or body aches

      • Headache

      • New loss of taste or smell

      • Sore throat

      • Congestion or runny nose

      • Nausea or vomiting

      • Diarrhea

  • Temperature may be taken upon arrival (forehead - touchless) and if taken, it must be less than 100.4 F (if you have 2 out 3 readings above 100.4 F, then you will be considered to have a fever)

  • Your appointment will be cancelled and you will be asked to leave the practice if we witness any severe "cold" or other illness symptoms listed above
