For your convenience, we offer prepayment online for gift certificates and/or account credit for yourself, friends, or family members.  They are available in $25 value increments.  If you wish to purchase a $250 gift certificate for others or account credit for yourself, then you will add 10 quantity of $25 (10 x $25) in your shopping cart to equal to $250 total. As an account credit, the amount purchased will never expire until it is used up for your procedure(s).
 
For a gift certificate, the recipient will only recieve one gift certificate in the amount of to the total value purchased.  Gift Certificate may only be redeemed by the original recipient (non-transferable) and must be picked up at our office by the purchaser or the recipient. Only one redemption per certificate. Expires in one year from date of purchase. Not valid to purchase with other offers, discounts, or certificates. No change will be given on redemption. Once gift certificate is redeemed, credit amount equal to certification amount will remain in recipient's account until the full gifted amount is used up prior to one year from the original purchase date.
